Job Description
Work Schedule: Monday-Friday 8:00am to 4:30pm, Occasional Weekend/Holiday
Principal Responsibilities and Tasks
The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.
Responsible for the first line supervision of a clinical treatment team of social work staff and students/ interns.A. Assists departmental management in evaluation of problem areas in clinical functioning. Makes recommendations regarding program and staff assignments to the department leadership.
Approves staff requests for leave and arranges for adequate clinical coverage when team members are absent. Arranges & orients employees for on-call coverage, as needed.
Provides performance management through regular contact and annual performance evaluations & implements performance improvement plan, as needed, through use of Just Culture approaches.
Provides clinical supervision of Social Work staff, including toward their clinical hours for Social Work licensure, in accordance with expectations outlined by the Maryland Board of Social Work Examiners.
As applicable: Provides field instruction for student interns. Serves as coordinator of field instruction and/or participates on field instruction task force.
Conducts regular meetings with team members to present new cases for assignment and treatment planning, review of ongoing cases, and discuss clinical and departmental issues.
Assigns cases to team members based on assessment of caseload size and clinical skills.
Assists team to develop initial treatment plans on new cases; coordinates presentation of ongoing difficult cases for additional team input.
Keeps statistics regarding volume of service, caseload size, admissions and discharges in order to have an equitable distribution of clinical work.
Provides clinical consultation to team members.
Coordinates orientation for new staff.
Provides advanced social work services to patients.
Develops & implements individual care plans.
Documents clinical interventions and patient’s progress in the patients medical record..
Conducts assessments and presents to clinical team for care coordination..
Provides primary case management working collaboratively with the patient, providers, other disciplines, payor, and patient’s support system to facilitate access to services that meet the patient’s needs.
Documents cost benefit analysis to assist in evaluating the plan of care.
Identifies needs in program planning and facilitates program development. Assumes leadership for developing new programs, research projects, clinical presentations at workshops, and clinical publications that relate in social work practice in healthcare.
Determines staff educational needs and develops/plan training programs for multidisciplinary staff and professional students.
Coordinates PI activities and implements recommendations pertinent to the team’s clinical area with the goal of improved care outcomes and reduced health cost.
Develops and maintains contact with various community service agencies to exchange information and facilitate delivery of services.
Participates in interdisciplinary meetings/conferences with appropriate medical, nursing and administrative staff as requested by departmental management.
Completes interviews for job applicants & participates in coordination of hiring process with management.
Qualifications
Education and Experience
Master’s degree in Social Work accredited by Council on Social Work Education (CSWE).
LCSW- C (Licensed Certified Social Worker-Clinical) licensure from the Maryland Board of Social Work Examiners.
Supervisory status through the Maryland Board of Social Work Examiners.
Minimum five (5) years of post-Master’s experience is required. One year of experience in a lead/supervisory capacity is required.
Knowledge, Skills and Abilities
Proficient knowledge of social work practices and case management is necessary.
Proficient leadership qualities are required, including ability to manage the work of Master’s level clinicians from the core disciplines and ability to provide emergency consultation to staff.
Proficient organization and problem-solving skills are required in order to develop/implement efficient work processes, and to successfully resolve difficult, conflict-oriented situations for the common good.
Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
Highly effective verbal and written communication skills are necessary in order to work effectively within the clinic setting.
