Job Description

General Summary

Under the supervision of a licensed social worker, coordinates resources through interdisciplinary collaboration to achieve optimal patient outcomes. Coordinates referral data and resources into a seamless model of access and care benefiting patients, participating physicians, and family members. Requires interaction with all customers (patients, physicians) in a courteous, helpful and compassionate manner.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.
Build and maintain relationships with multidisciplinary team to build patient referral mechanisms and promote patient navigation services.
Help patients move through the healthcare system by assisting with practical problem solving related to concrete needs, such as lodging and transportation.
Complete comprehensive assessments of the barriers to care that contribute to disparities and identify information, resource and emotional support needs of newly diagnosed and medically underserved patients, survivors, and caregivers.
Establish and leverage relationships with local service providers and other healthcare providers to enhance service and referral networks.
Implement a follow-up plan for each patient to ensure that services have met their needs.
Provide support to special projects and assignments.
Performs other duties as assigned.

Qualifications

Education and Experience

Bachelor’s degree required, preferably in health care related field and
1 year equivalent experience in oncology, social work, community health education or related field required.

Knowledge, Skills and Abilities

Self-directed and able to organize and manage multiple tasks/projects simultaneously.
Ability to promote programs and services to community.
Understanding of referral processes to expedite patient care.
Proficient in review of medical records and other patient care information.
Excellent customer service skills.
Demonstrated skills in verbal, written and electronic communication.
Ability to work with a variety of health-related computer systems.
Ability to work within a team of multidisciplinary care providers.

Pay Range: $24.52-$34.84
