Job Description
On a daily basis the Case Manager Team Lead will ensure safe, timely and appropriate provision of services. Coaches team members to problem solve cases to optimize patients’ health, reduce unnecessary care and limit healthcare spending. Monitors team member documentation and data collection to ensure complete, accurate and compliant records. Works collaboratively with the Population Health management team, as well as internal and external stakeholders. May interact with patients or customers ranging in age from infant to geriatric. Position requires coverage on holidays and on back- up call system on weekends.
Qualifications
Education & Training: Current RN license required OR Current Social work license LMSW, LCSW, LCSW-C required. Associate’s degree or diploma in nursing OR Master’s in Social Work degree required. Bachelors of Science in Nursing preferred.
Work Orientation & Experience: Two years of experience in Case Management and understanding of Utilization Management preferably in an acute care setting required.
Skills & Abilities:
Demonstrate skill in a) clinical case management; b) data analysis; c) effective critical thinking; d) written and oral communications; and e) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult). Demonstrate knowledge of a) acute care criteria and b) case management principles. Demonstrate ability to a) communicate and collaborate effectively with both internal and external customers (colleagues, Medical Staff, liaisons, and patient/family); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, and ethical standards; d) prioritize work assignments and manage time effectively to complete duties; and e) be available to adapt hours to meet the needs of all customers.
